The first field must contain your email address.

The legend of each input field can be edited. An empty legend causes the deletion of the input field in the form. Each field can be of one ore several lines et can be made required or not. The first two fields are dedicated to an emeil address and the object of the message. 

“Header copy” and “Foot copy” are texts that frame the copy of the message the visitor will receive: an expression of thanks, your signature, etc.

The form takes the width  it is allocated by the browser on the page; to reduce it to a more useful size, it is possible to assign an alignment "include below on the left" or "include below the right”
